  Light a personalized candle in memory of a deceased loved one for one year.
  
  Located as you enter the main doors of the Church on the left wall.
  
  If you would like to request a personalized
  plaque for a Memorial Candle,
  please complete the form below.
  
  The plaque will honor your loved one for one year
  After that time, you may renew the plaque for an additional year.
  The requested donation is $100 per plaque for the initial year
  and $40 per plaque for each year there after.
